The position of Community Advocacy Project (CAP) Coordinator is an important one within our Domestic Violence services program. Our ideal candidate for CAP Coordinator is someone who is comfortable with public outreach and providing presentations, working with and supervising our group facilitators and interns, as well as providing services to our clients.
CHILDREN’S SUPPORTIVE CARE MANAGER
Children’s Supportive Care Managers are skilled at coordinating the care that our young clients need to maintain their health and life goals. Our ideal candidate for Children’s Supportive Care Manager is someone who has a minimum of 2 years of relevant experience with SED/SPMI or chronic physical health conditions; experience with linkage and coordination of community services and emergency services; experience in substance abuse treatment is also helpful.
FAMILY ASSESSMENT / FAMILY SUPPORT WORKER (POSITION BASED OUT OF ORLEANS COUNTY, NY)
Our Healthy Families program focuses on the period from pregnancy through a child’s fifth birthday. A Family Assessment/ Family Support Worker will develop a relationship with the family, help to identify areas of need, and work with the family on an on-going basis to coordinate support and work towards the family’s health and life goals. Our ideal candidate for the role of Family Assessment / Family Support Worker is someone who can help to coordinate resources, is articulate, and someone who can establish an on-going relationship with client families.
