CHILDREN’S SUPPORTIVE CARE MANAGER

QUALIFICATIONS
Associate’s Degree or Bachelor’s Degree in Social Work or related.  Minimum of 2 years specialized training and experience with SED children, adolescents and their families; experience with coordination of community children’s services, emergency services, family court & legal issues, and education system. Valid NYS driver’s license. Use of personal vehicle during work hours.

JOB RESPONSIBILITIES:

  • Provide direct social work contacts to 25- 30 children and their families
  • Provide HH services for children with low, medium and high acuity needs
  • Assist families and children in the acquisition and maintenance of public benefits e.g. financial, educational, social and community services
  • To complete required paperwork such as but not limited to CANS-NY assessments (update when required), family- driven Plan of Care (POC), intake assessment(s), etc.
  • Maintain consistent and on-going contact with client, families, providers, community resources, school and any other services the child is linked with while fulfilling unit of service expectations
  • Facilitate linkage to appropriate services; coordination of needed services and/or existing relevant services to the POC
  • Assure clients has access to the agency’s on call services for assistance after hours
  • To provide on call services after hours as scheduled
  • Prepare professionally accurate and updated case documentation
  • Progress notes to be completed within Health Home guidelines of all case activities (outreach, consent, assessment, face to face, service coordination, etc.)
  • Maintain CANS-NY certification, complete the Mandated Reporter training annually
  • Provide education to family and child concerning the nature and desired effect of services, along with information concerning conditions being treated
  • Assure responsible transition of client service into and out of the HH care, between child and adult HHs and between inpatient and community care as appropriate
  • Establish working relationships with referral sources and other related service agencies
  • Verify clients Medicaid status on a monthly basis or as frequently as needed
  • Meet with Program Supervisor on a regular, planned basis
  • Participate in mandatory and optional training, including but not confined to : CANS-NY administration, family-driven youth guided, trauma-informed care, IT, LGBT, safety in community work and meeting management and staff meetings
  • Convene and conduct quarterly meetings to review progress, update POCs and confirm continuing client eligibility
  • Review current practices to advocate for change when needed, particularly those needed by families and minority populations
  • Perform other duties as assigned by Administration
