POSITION: Supportive Case Manager
QUALIFICATIONS: Associates Degree or Bachelor’s degree in Social Work or related field; 1-2 years specialized training and experience with SPMI or mentally disabled population; experience with linkage and coordination of community services and emergency services; valid NYS driver’s license and use of personal vehicle during work hours
ACCOUNTABILITY: Reports to Supervisor, Case Management Services
DUTIES:
Provide direct social work contacts to at least 30 SPMI consumers
Maintain consistent and on-going contact with consumers and families while fulfilling unit of service expectations
Conduct initial intake interview and needs assessment to determine treatment goals
Prepare professionally accurate and updated case documentation
Prepare statistical and administrative reports on a timely basis
Provide consistent, supportive client contact; emotional support; daily living counseling; legal advocacy; financial counseling; representation regarding rights and entitlements
Facilitate linkage to appropriate services; coordination of hospitalization admission and discharge; mental health education to clients, families, and community
Assure client linkages to services for 24 hour coverage
Establish working relationships with referral sources and other related service agencies
Review current practices to advocate for change when needed, particularly those needed by families and minority populations
Team with billing staff to ensure client active Medicaid status and accurate, complete client billing
Meet with Program Supervisor on a regular, planned basis
Attend and participate in appropriate training and staff meetings
Advocate for cultural diversity, as appropriate
Report all suspected incidences of child abuse/neglect to NYS Hotline with supervisory approval
Comply with all Policies and Procedures of FACS (including NASW Code of Ethics, Collective Bargaining Agreement, and Corporate Compliance Policies) and all governmental and legal requirements specific to the program

POSITION: Family Support Worker, Healthy Families Niagara
QUALIFICATIONS: High School Diploma or Equivalent, AAS Degree in a Health and Human Service field preferred. Live near or in target area, maturity and experience in working with families and children, knowledge of normal child growth/development and parent-child relationships, ability to relate to families from a strength based, family centered model, available to families during non-typical work hours, knowledgeable about community resources, ability to establish and maintain personal and programmatic boundaries while providing services, believes nurturing non-violent discipline of children, experience with culturally diverse communities and families, good writing and organizational skills, experience in home visiting preferred, a valid NYS Driver’s License is required and use of personal vehicle during work hours.
ACCOUNTABILITY: Reports to a Program Supervisor.
DUTIES:
Demonstrates motivation and the ability to learn and practice basic supportive skills
Initiates and maintains regular and long term contact/support with families.
Conducts visits with a family centered and strength based approach
Assists families in strengthening the parent-child relationship, optimizing home environment, improving the family support system, and establishing goals
Advocates for families and their children while teaching them to advocate for themselves
Identifies and refer for contact/appointments at other supportive agencies, including health care appointments
Conducts assessments of normal growth and development of the target child
Completes referral forms and other agency forms/reports as necessary
Attends case conferences with Program Supervisors and Family Assessment Workers
Attends and participates in core and wrap around trainings and team and staff meetings.
Meets with his/her supervisor at a minimum of two hours per week for supervision at a regularly scheduled time.
Assists with participant Support Groups, Parenting Classes, obtaining donations and any other projects assigned by your Supervisor.
Assists in presenting Healthy Families Niagara and FACS Agency services to the community at large.
Reports all suspected incidences of child abuse/neglect to appropriate authorities with supervisory approval.

POSITION: Casey House, Resident Manager
QUALIFICATIONS: A minimum of a B.A or B.S degree in a Human Services area and one – three years experience in a human service program, preferably in a residential setting. Position requires flexibility in scheduling with occasional weekends. Valid NYS Driver’s License and reliable transportation necessary. NYS Division for Youth (Office of Children and Family Services) regulation mandates that all direct service staff have a yearly physical and be checked through the NYS State Central Registry for Child Abuse and Maltreatment (NYSSCR). Your completed physical form must be received by your direct supervisor within thirty (30) days of hire.
ACCOUNTABILITY: Reports to Program Director
DUTIES:
Supervise all Resident Care Workers.
Meet with all Resident Care Workers on a regular, planned basis, including monthly Resident Care Worker group meetings.
Prepare schedule for Resident Care Workers, responsible for ensuring adequate coverage for all shifts.
Provide case management to youth, on an as needed basis, as directed by the Program Director.
Rotate with other program staff to provide on-call 24/7 emergency coverage.
Assist the case manager in providing 30, 60 and 90 day follow up services to former clients.
Purchase food and supplies for shelter program. Post menus and keep (one year) records of past menus. Order and pick up food supplies from W.N.Y. Food Bank with appropriate records maintained. Conduct Weekly Perishable food shopping.
Insure all intake procedures are maintained.
Schedule annual fire inspections and ensure that shelter is in compliance with Fire Safety Codes.
Direct all maintenance needs at the shelter to ensure a healthy and safe environment for youth in residence.
Be available to youth to discuss their concerns regarding residence at Casey House.
Establish working relationships with referral sources and related service agencies.
Upkeep all program forms and intake packets.
Maintain records for all residents referred by New Directions, for Respite Services and bill them for services.
Team with program staff to provide consistency of services.
Meet with and participates on a regular, planned basis with Program Director for supervision.

Maintain fiscal responsibility for cash expenditures of food and supplies.

Represent FACS and the program as appropriate in community.
Provide program related clerical duties, including staff schedules, correspondence, filing and RHY-MIS reporting.
Answer incoming calls from the public in a polite and welcoming manner, as appropriate in teaming with other staff on duty. Assess crisis situations and take appropriate action. Conduct intakes and admit youth into shelter.

POSITION: Relief Worker, PASSAGE SHELTER
QUALIFICATIONS: Experience in human services and the ability to work in a crisis intervention setting. Must have reliable transportation and valid NYS Driver’s license. Must be willing to work afternoons, midnights, weekends, and Holidays for hourly reimbursed, shift work.
ACCOUNTABILITY: Reports to Residential Services Coordinator
DUTIES:
Schedule shift work in cooperation with other Relief Staff to cover evening, midnight, weekend, and Holiday hours with occasional coverage of some day hours, as needed.
Punctuality and consistency in reporting for and covering scheduled work shifts.
Answer incoming telephone calls from domestic violence victims, assess crisis situations, and take appropriate action. Answer calls from general public requesting information. Handle all callers in a polite and welcoming manner.
Conduct client Intakes (including all paperwork in legible manner) and admit clients to Shelter. Respect client confidentiality.
Follow all appropriate procedures upon arrival of client to be admitted, including orientating clients to Shelter physical plant and amenities, educating clients on the Shelter Guidelines and other rules/procedures, clearly presenting fire safety plan, and accompanying client to their assigned room (provide toilet articles).
Documentation of Daily Log notes and sharing other client information with incoming coverage staff, as appropriate.
Maintain contact with Shelter clients, providing brief emotional support, as needed. Refer clients to social work staff for counseling issues.
Monitor and oversee daily chores, meal preparation and cleanup, and ensure that appliances are used in a safe manner.
Maintain a safe, comfortable environment for women and children in residence, including not leaving the residence unsupervised without supervisory approval.
Assist Residential Services Coordinator in obtaining and storing supplies, organizing donations, and in maintaining Shelter building and grounds.
Transport clients on occasion, as directed by supervisor.
Accompany client on police escort or to the Department of Social Services or other agency appointments, as directed by supervisor.
Participate actively in regularly scheduled supervision and in Shelter staff meetings.

DUTIES:
Maintain a safe and comfortable environment for runaway and homeless youth in residence.
Answer incoming telephone calls from the public, assess crisis situations and take appropriate action in a polite, welcoming manner.
Conduct intakes and ensure that all admission procedures are completed in a thorough and legible manner.
Maintain confidentiality of all client information, identity, and documentation.
Introduction and orientation of youth to all residential procedures including HIV/AIDS Precautions, Child Abuse Recourse, Fire Safety, Resident Guidelines and other assigned tasks.
Sustain punctuality and consistency in reporting for scheduled work shifts.
Awareness of the whereabouts and safety of all youth in residence at Casey House at all times.
Supervise and conduct recreational activities for youth in residence.
Refer youth to case manager for any life planning/emotional issues.
Quality completion of staff chores and housekeeping duties as required/assigned.
Accurate completion of Daily Log Notes and reporting of significant youth behavioral concerns to supervisor or other appropriate staff.
Nightly documentation of youth behaviors in individual case files, including any calls made or received significant to youth’s case.
Monitor and supervise daily chores, meal prep and cleanup; assure appliances are used in a safe manner by anyone in the residence.
