Mental health and other chronic illnesses present many challenges for children and adults in the community, along with their family members. Family & Children’s offers a variety of services to aid both children and adults in their recovery efforts. Once enrolled in the program, individuals (and families) will work with a Care Manager who will support and assist them in navigating the sometimes confusing and complicated healthcare and social service systems to access the services they need to stay healthy and to improve their quality of life.
The heart of our service is building a “supportive relationship” with the people we serve, helping them find the right care and services. A Care Manager can help you navigate, learn about services and get connected to the services you need. There is no cost to the consumer and services are offered on an outreach basis.
